  • We coordinate the transition from hospital to home. This includes mobility aids, home setup, caregiver support, and rehab planning to avoid delays in recovery.

  • We assess a client’s ability to manage daily personal care tasks. Our reports support PCA allowance decisions with clear rationale and itemized support needs.

  • We build safe, step-by-step return-to-work plans that match medical recovery, job demands, and client readiness. We work closely with employers and case managers to align expectations.

  • We assess the physical, cognitive, and sensory demands of the job on-site. Our reports help clarify whether a return to the same role is safe and what supports may be needed.

  • We break down the physical and cognitive tasks of a job. This supports return-to-work planning, functional matching, and medical-legal file clarity.

  • We provide structured treatment for symptoms like fatigue, concentration issues, and memory loss. Programs include education, pacing, and task retraining.

  • We provide objective measurement of functional limitations related to scarring or joint injuries. Reports support claim decisions and long-term impairment estimates.

  • We assess the need for wheelchairs, seating supports, or mobility aids. Our recommendations focus on safety, independence, and alignment with ICBC equipment funding criteria.

  • We assess a client’s current functional abilities and compare them to specific job demands, using employer reports, JSV findings and National Occupation Classification (NOC) guidelines. Reports include objective findings on lifting, endurance, posture tolerance, and overall work readiness.

  • We compare current abilities to the job role. This helps determine if a client is ready to return, needs accommodation, or should consider alternate roles.

  • We coordinate treatment, communicate with your team, and ensure timely progress updates. We guide clients through each rehab stage with continuity and structure.

Our occupational therapists will conduct a holistic assessment of the impact of the accident on your physical, cognitive, and psychosocial function, while considering your home/work and community environments and the impact of those on your recovery. They will make recommendations for treatment and services, in addition to arranging delivery and installation of home adaptive equipment.

Hospital Discharge Planning:

Our Occupational Therapists work seamlessly with the hospital discharge team and family members to ensure a smooth transition to a safe home with the needed support. We aim to minimize any risk of re-injury or other complications.

Personal Care Assessments:

Personal Care Allowance assessments help to determine services, and hours needed, for the client to get their personal needs met within the home. We provide these to those who are physically or psychologically impaired.

Functional Re-Training:

After safely returning home, our team is available to help the client achieve their desired OT results. Our therapists will help individuals with their desired outcomes, such as:

  • Increasing their ability to safely and independently perform daily activities

  • Improving their physical condition through functional training

  • Enhancing their cognitive abilities through training and compensatory techniques

  • Returning to work

  • Regaining the ability to live their life as they did before injury or illness

Case Management:

Functional Therapy & Wellness provides case management and rehabilitation consulting to individuals who have recently suffered catastrophic injuries or are having difficulty returning to their previous level of functioning. Our goal is to work with the referral source and other important parties to ensure that the individual has access to the most suitable and cost-effective support and services.
